1. Acceptance of Terms

These Terms and Conditions govern access to and use of the CareLink Nebraska public website. By using the website, you agree to these terms and the Website Privacy Policy.

Do not use the website if you do not agree with these terms.

2. General Information Only

Public website content is provided for general informational purposes. It does not constitute a diagnosis, treatment recommendation, prescription, individualized medical advice or guarantee that a particular service is appropriate for you.

Do not delay or disregard professional medical advice because of information displayed on this website.

3. Emergency and Crisis Situations

CareLink Nebraska’s website and routine telehealth services are not emergency services.

Call 911 or seek immediate emergency care for chest pain, severe breathing difficulty, stroke symptoms, severe bleeding, loss of consciousness, sudden severe pain or another potentially life-threatening condition.

Call or text 988 for suicidal thoughts, thoughts of self-harm or an emotional or mental health crisis.

4. No Provider-Patient Relationship Through Website Use

Visiting the public website, reading its content, sending a general contact form or receiving an administrative response does not by itself establish a provider-patient relationship.

A provider-patient relationship may be established only through CareLink Nebraska’s approved patient process and subject to eligibility, informed consent, provider acceptance and applicable law.

5. Eligibility and Service Area

Service availability may depend on your physical location at the time of care, age, clinical circumstances, provider licensure, appointment availability and applicable law.

Descriptions of service areas do not guarantee that every visitor is eligible for care.

6. Telehealth Limitations

Telehealth has inherent limitations. A provider may be unable to perform a physical examination, obtain vital signs, conduct immediate laboratory testing or use diagnostic imaging during a virtual appointment.

A provider may recommend primary care, urgent care, emergency care, laboratory testing, imaging, specialist evaluation or another in-person service.

Treatment, referrals and medication decisions remain subject to professional judgment and applicable law. Medication or refill requests are never guaranteed.

7. Plans, Pricing and Payments

Public pricing is intended to describe available plans generally. Final enrollment materials should specify:

• Current fees
• Payment schedule
• Included and excluded services
• Renewal and cancellation rules
• Refund terms
• Eligibility requirements
• Patient responsibility for outside services

Services not expressly included in a plan may result in separate charges. Laboratory work, prescriptions, imaging, emergency services, specialists and outside facilities may charge separately.

CareLink Nebraska membership should not be described as health insurance unless it is legally structured and licensed as insurance.

8. Scheduling and Communications

Appointment requests are not confirmed until accepted through the approved scheduling process. Availability may change.

Users are responsible for providing accurate contact information and monitoring communications related to scheduling.

General email and public contact forms should not be used for urgent requests or private medical information.

9. Acceptable Use

You agree not to:

• Use the website for unlawful or fraudulent activity
• Attempt unauthorized access to systems or accounts
• Transmit malicious software or harmful code
• Interfere with website security or availability
• Impersonate another person
• Submit false or misleading information
• Scrape or copy website content unlawfully
• Use the website to harass or threaten others

Website text, branding, graphics, layouts, icons and other original materials are owned by or licensed to CareLink Nebraska unless otherwise stated.

You may view and print reasonable portions for personal, noncommercial use. You may not reproduce, republish, sell, modify or distribute protected website materials without permission.

11. Third-Party Services

The website may link to third-party scheduling, payment, laboratory, mapping or patient-care services. CareLink Nebraska does not control every third party’s content, availability or practices.

Third-party services may be subject to separate terms, privacy policies and agreements.

12. Website Disclaimers

To the extent permitted by law, the public website is provided on an “as is” and “as available” basis. CareLink Nebraska does not guarantee uninterrupted access, complete accuracy, freedom from technical errors or that every service will remain available.

Nothing in these terms excludes obligations or rights that cannot lawfully be excluded.

To the extent permitted by applicable law, CareLink Nebraska will not be liable for indirect, incidental, special, consequential or punitive damages arising solely from use of, or inability to use, the public website.

To the extent permitted by law, you agree to be responsible for losses arising from your unlawful misuse of the website, violation of these terms or infringement of another person’s rights.

These website terms are governed by applicable federal law and the laws of the State of Nebraska, without regard to conflict-of-law principles, unless another law must apply.

The final dispute-resolution and venue provisions should be reviewed by Nebraska counsel before publication.

CareLink Nebraska may update these terms when the website, services, business practices or legal requirements change. Updated terms become effective when posted with a revised effective date.
